INSTRUCTIONS
Follow these
guidelines carefully to complete the application for DV-2002.

1- Applicant's Full
Name: Last
Name (surname/family), First Name and Middle Name
|
2- Applicant's Date
and Place of Birth:
Date of birth must be written in this format:
Day/Month/year. Spell out the month -- Example:
15 June 1970
Place of birth:
City/Town, District/Province, Country -- Example: Munich, Bavaria,
Germany.
The name of the country should be that which is
currently in use for the place where the applicant was born (Slovenia,
rather than Yugoslavia; Kazakstan rather than Soviet Union, for
example).

3- Applicant's native
country, if different from country of birth:
If the applicant is claiming nativity in a country other than his or her
place of birth, this country (instead of the country of birth)
must be listed. Nativity in most cases is determined by the applicant's
place of birth. However, if a person was born in an ineligible country
but his/her spouse was born in an eligible country, such person can
claim the spouse's country of birth rather than his/her own. Also, if a
person was born in an ineligible country, but neither of his/her parents
was born there or resided there at the time of the birth, such person
may be able to claim nativity in one of the parents' country of birth.

4- Name, Date and
Place of Birth of Applicant's Spouse and Children (if any):
you must list your spouse, that is husband or wife, and all children who
are unmarried and under age 21 (born after September 30, 1980). You must
list your spouse even if you are currently separated from him/her.
However, if you are formally divorced, you do not need to list your
former spouse. You must list all your unmarried children under age 21,
whether they are your natural children, your spouse’s children by a
previous marriage, or children you have formally adopted in accordance
with the laws of your country. Even children in the above categories not
currently living with you must be listed on the entry.
A husband and wife
may each submit one lottery application. If you receive a Green Card
through the lottery, your spouse and unmarried children under 21 will
also get Green Cards at the same time. All children over 21 must file
separate applications. Submitting
separate applications for you and your spouse double your chances of
winning.

5- Applicant's
Mailing Address: on the application we
use our mailing address. Using our address is very helpful for
people who 1) want to protect their privacy 2) may change their mailing
address in the future 3) the mailing system in their country is not
reliable or slow.
You must provide us
with your complete address and contact information. Since it will
be to that address we will send the notification letter if you win.
Our office will contact all winners and send their notification letters
by express or priority mail.

6- Applicant's
Signature: The applicant must sign
the application using his/her normal signature. Failure to personally
sign the application will disqualify you.

7- Photograph:
Include a recent "passport style" photograph (37mm x 37mm)
color or black & white with your name written on the back. Please, do
not attach the photo to the application.
- Photograph dimension
should be 37mm x 37mm or 1-1/2" x 1-1/2" inches.
- Photograph must be
taken recently.
- Photograph must have
your name on the back-- close to the photo center.
The photograph of the
principal applicant only is required. However, a married couple
submitting two separate applications must send two photos (one for
husband, one for wife). Remember if you submit two separate
applications, for husband and wife, you double your chances.
